Wellness Exams & Medical Forms

• Each time your child gets a well child exam or immunizations, a health appraisal form and an immunization certificate should be updated and returned to the Center.
• If any medication needs to be administered to your child while they are at the CCC, we need a Permission to Administer Medication Form filled out and on file with us.
